Hello! My head is 22″ – this description says “Small/Medium”. Does that mean there is a string on the inside that allows it to be tightened?

Byron Bay does not have string inside that allows it to be tightened but the hat will be delivered with a sizing tape that could be put under the inside sweat band to make the hat fit more comfortably.
